What is an ACL repair?

ACL repair is a surgical treatment aimed at fixing a torn anterior cruciate ligament (ACL) by reattaching the native ligament to the bone. While ACL reconstruction – using a tendon graft to replace the torn ligament – has been the gold standard for many years, renewed interest in ACL repair has emerged thanks to advances in surgical techniques and research. 

ACL repair may be appropriate for certain ACL tear types, particularly proximal or femoral-sided ACL tears, where the ligament is torn near its attachment on the thigh bone (femur). Unlike ACL reconstruction, which uses an autograft or allograft tendon, ACL repair preserves the patient’s original ligament, potentially maintaining better knee proprioception and a more natural knee feel. 

What’s involved in an ACL repair procedure?

ACL repair is performed arthroscopically (keyhole surgery) under general anaesthetic. The torn ACL is reattached to its femoral insertion using sutures and reinforced with a synthetic internal brace (tape) to protect the ligament during healing. This minimally invasive procedure focuses on preserving the native ligament structure.

What does the recovery involve?

Post-operative recovery typically involves protected weight-bearing with a knee brace during early healing. Physiotherapy starts soon after surgery to restore knee motion, reduce swelling, and progressively rebuild strength and stability. While early recovery may be faster than ACL reconstruction, returning to sport generally takes a minimum of 6 months, depending on individual progress and sport-specific demands. Strict adherence to a tailored rehabilitation program is essential for optimal outcomes.
