Please complete the new patient form prior to your consultation with Dr Free. You may complete the form online here or download the PDF version here and return it via email to: admin@matthewfree.com.au. Submission of this form is required before your appointment.
During your consultation, Dr Free will conduct a thorough physical examination. Please wear loose, comfortable clothing that allows easy access to the affected area for examination purposes.
We kindly ask that you arrive at least 15 minutes prior to your scheduled appointment time to complete registration and any outstanding paperwork. We respect your time and will make every effort to begin your consultation promptly. In the event of any unexpected delays, we will contact you in advance and provide timely updates.
All consultations incur an out-of-pocket fee, payable at the time of your appointment. Medicare rebates are available and will reimburse a portion of the consultation cost. Our administrative team is available to assist you with processing your Medicare claim and any related queries.
If surgery is recommended, we will provide clear and transparent information about all related costs. Before your procedure, you will receive informed financial consent detailing the anticipated fees, so you can make decisions with confidence.
We accept payment via EFTPOS, Visa, MasterCard, and bank transfer. Full payment for consultations is required at the time of your appointment. For surgical procedures, payment details and timelines will be discussed during the financial consent process. Please contact our administration team if you require assistance with payment arrangements.
To schedule an appointment, please call 0460 017 816 or email admin@matthewfree.com.au. Please bring your GP referral with you.
Dr Free prioritizes timely access to care and aims to see all patients as soon as possible. Urgent appointments are available when clinically necessary.
A gap fee may apply. This is the difference between the total cost of your surgery and the amount covered by Medicare and your private health insurance.
Because the rebates from Medicare and health funds have not kept up with rising healthcare costs, Dr Free charges a gap fee to help cover the difference. The exact out-of-pocket cost depends on your health fund and the complexity of your surgery.
You will receive a detailed fee estimate before surgery, and Dr Free’s fees will always stay within the Australian Medical Association (AMA) recommended rates. Payment is required one week before surgery, and our team will guide you through the financial consent process to ensure you understand all costs.
